It also means following the resident care plans and respecting the instructions in place. For example ,we need| | | | |to be sure a resident is turned every two hours, applying Cavilon cream on areas; we fill in turning charts. Under | | | | |the duty of care, we as a carer assistant must always be aware of and raise concerns regarding possible pressure | | | | |areas. Pressure area care may be provided by a team that is made up of | | | | |different types of healthcare professionals. This type of team is known as a multidisciplinary team (MDT) made up by | | | | |a nurse,dietitian, physical therapist,an occupational therapist,and in our case health carer assistants. By working | | | | |in a team all concerned areas can be tackled.For example after the risk assessment is completed a nurse can regularly| | | | |come in the house to dress the areas, we as health care assistants turn regularly the resident, monitor fluid and | | | | |diet intake, raise concerns,etc. Team working plays a vital role in making sure a resident is well looked after and | | | | |pressure areas are managed accordingly. | | | | |It is important to follow the agreed care plan because it has been drawn to provide the best care required. Not | | | | |following these the law is broken and residents are neglected, trust being breached. | | | | |I would inform my senior, head of care or manager of any concerns related to the agreed care plan and always record | | | | |information in care plan. | | | | |The purpose of pressure ulcer risk assessment and grading tools are to help assess a patient’s potential | | | | |. Pressure relief equipment may include cushions, air or foam mattresses, bed cradles and joint protectors. | | | | |To reduce the effect of friction and shear on the skin, appropriate manual handling techniques and equipment need to | | | | |be used. Slide sheets and hoists can be useful to reduce shear when transferring or repositioning although they | | | | |should not be left underneath the client. | | | | |Some barrier creams and oils may be recommended as a preventative measure, or to aid in healing of an existing sore. | | | | |It is always advisable to make sure that all devices and equipments are in good working order.
