Unit 204: Principles of safeguarding and protection in health and social care. Outcome 1 1.1 Physical abuse Definition: Physical abuse could involve, contact intended to cause feelings of intimidation, injury or other types of physical suffering or harm. Sexual abuse Definition: Sexual abuse is forcing undesired sexual behaviour by one person onto another. Emotional/psychological abuse Definition: Could involve threats or actions to cause physical or mental harm, humiliation or isolation. Financial abuse Definition: Is the illegal or unauthorised use of a person’s money, property or other valuables. Institutional abuse Definition: Involves the failure of an organisation to provide professional and appropriate services to vulnerable people. Self-neglect Definition: Is a behavioural condition, were the individual neglects to attend to their basic needs. Neglect by others Definition: Is a passive form of abuse were the perpetrator is responsible for the care of someone unable to care for themselves. 1.2. Physical Abuse Signs/symptoms: * Injuries in the shape of objects. * Unexplained weight changes. * Changes in behaviour. * Medication being lost. Sexual Abuse Signs/symptoms: * STD’S / Pregnancy * Bruises around genital areas * Someone trying to take control of body image, i.e. anorexia, bulimia, self-harm. Emotional Abuse Signs/symptoms: * Low self esteem * Sleep disturbance * Increased incontinence Financial Abuse Signs/symptoms: * Unusual transactions. * Bills not being paid. * Loss of assets Institutional Abuse Signs/symptoms: * Treating adults like children. * Lack of choices. * Lack of privacy & dignity. Self Neglect: Signs/symptoms: * Lack of personal care. * Untreated medical conditions. * Continuous weight loss. Neglect by others: Signs/ symptoms: Physical abuse signs could be burn marks, bruising in unusual places, changes in the person’s behaviour, becoming withdrawn. Sexual abuse could include self-harming, reacoring urine infections, depression. Inability to form relationships, difficulty walking or sitting. Emotional abuse could be loss of self confidence, withdrawn. Financial abuse, unpaid bills, basic needs not being met, lack of cash on a day to day basis, loss of trust. Institutional abuse, inability to make choices or decisions, agitation if routine is broken, disorientation. 1.3 A person who has low self esteem and no sense of self worth. Someone who has been physically and emotionally abused in the past. Someone that has been sexually abused. People who have substance abuse problems. The mentally ill or homeless. Someone who is mentally handicapped or medically dependent people, such as nursing home patients or people with debilitating diseases like MS, Parkinson's, or cancer. The elderly are at risk as are children. This is because they have nobody to protect them and are often abused by people in their lives. Outcome 2 2.1 You should ensure that the welfare of the person whom the abuse has taken place against, also the interests of the person who the allegations have been made against are also protected. 2.2 Make careful records of what was said using the persons own words as soon as possible. Date, time and sign this as it may need to be used in any investigation. 2.3 Some of the ways in which evidence can be preserved are taking of signed and dated witness statements, photo graphic evidence been taken, making sure that all factual evidence e.g. bank statements, financial records are stored away securely. Any electronically stored evidence is stored on a password protected system. Outcome 3 3.1 No secrets (2000). The Commission for Social Care Inspection (CSCI). (2000). Mental Capacity Act (2005). 3.2 1) Criminal records bureau (CRB). Assess the suitability of people working with venerable people. 2) Independent Safeguarding Authority (ISA). Work closely with the CRB when undertaking the vetting process or reaching a decision to put a person on the barring list. 3) Local authorities/NHS play a key role in assessments, investigations and protection planning. 3.3 1) The laming report into the death of Victoria Climbie (2000) 2) Birmingham Social Services review into the starvation and death of Khyra Ishaq (2010) Outcome 4 4.1 * Working with person-centred values The key values are dignity, privacy, promoting independence, choice and rights. Decreasing vaunrabilaty by increasing self confidence. Letting the individual make their own choices and decisions, listening to the individual. Promote that all adults have the right to live their lives free from violence, fear and abuse, and that we all have the right to independence. * Encouraging active participation Could help decrease the likelihood of abuse by encouraging the Individual to take part in activities and encourage personal care. Also by improving self-confidence, self-awareness and promoting involvement could all help reduce the risk of vulnerability * Promoting choice and rights Would help decrease the likelihood of abuse through promoting the individual to make choices and to make their own decisions. Helping them to make them self’s less vulnerable by promoting independence and the importance of consent. 4.2 It is a legal requirement to have this in place; it is also a way of following up any complaints. As having it available could help reduce the risk of abuse Outcome 5 5.1 Neglect in the duty of care in relation to personal care, washing, toileting, dressing etc. Unreliable systems for dealing with monies, such as failing to record accurately, miss use of power of attorney. Inappropriate communications or sharing of information e.g. failing to keep up to date with safeguarding issues and ongoing training, breaching confidentiality. 5.2 It is important to report unsafe practices that you identify, to your manager or senior. As soon as you observe them, you should do this verbally and in writing. 5.3 If no action has been taken following a report of suspected abuse. You have the right to go direct to social services with your concerns or to the police. You will be protected from the risk of reprisals for reporting any concerns.
