Medication 3.1. Describe the roles and responsibilities of those involved in prescribing, dispensing and supporting use of medication. The Doctors role is to determine what medication the patient needs. It is his responsibility to prescribe the correct medication, the dose, the type and amount. The chemist’s role is to dispense the medication that is written on the prescription, it is their responsibility to make sure that the items they give out are correct and exactly the same as what is written on the prescription. If there are any mistakes they must contact the doctor. The carer’s role is to make sure that the patient gets their medication. It is their responsibility to give the patient the correct medication, at the correct time with the correct dose. Everything that is given out must match what is printed on the Mar sheet and must be signed for. 3.2. Explain where responsibilities lie in relation to use of ‘over the counter’ remedies and supplements. If I saw a client taking over the counter remedies I would ask where they got them from and why they needed to take them, I would then inform the manager and document it in the careplan. If the client was not capable of telling me I would remove the remedy and inform the manager again. 4.1. Describe the routes by which medication can be administered. Orally Injection Intramuscular Intravenous preparation Transdermal preparation Sublingual (under the tongue) Rectal Epidural Subcutaneous administration Inhalant 4.2. Describe different forms in which medication may be presented. Tablets Capsules Sprays Creams Liquid Powder IV solution 4.3. Describe materials and equipment that can assist in administering medication. Measuring spoons Pill pots Syringes Medication trolley Water jug MAR sheets 6.1. Explain the importance of the following principles in the use of medication: Consent: The client must consent to taking the medicine, you cannot force anyone to take something that they don’t want to and you must never hide medication in food etc. If the client does not have the mental capacity to consent then the family or doctor must be informed. Self medication or active participation: The client has a right to decide if they want to be self medicating or take an active part in taking their own medication, this assists with maintaining their dignity and independence and is extremely important. Dignity and privacy: I must ensure that the client’s dignity and privacy is respected at all times. E.g., don’t ask if someone wants laxatives in front of other people, they may not want other clients, friends etc to know what medication they are taking and what it is for. Confidentiality: I must not discuss a client’s information/records with other clients etc, it is only to be discussed with the client, doctor and people directly involved with the clients care. 6.2. Explain how a risk assessment can be used to promote individuals independence in managing medication. A risk assessment is used to find out what risks (if any) there may be if the client wants to manage their own medication. The risk assessment will identify if the client is capable of remembering when to take the meds, which medication to take, what dosage etc and it will also identify if there will be any other risks to other clients or staff. The risk assessment must be kept up to date and regularly reviewed. By carrying out an assessment you will be able to give the client more independence by letting them have all or some control over their medication. Unfortunately the risk assessment might also show that it is not in the client’s best interests to manage their medication. 6.3. Describe how ethical issues that may arise over the use of medication can be addressed. I.e. A client is refusing to take medication as they believe they are being poisoned. I would follow company policy and report to my senior in charge/manager and would document everything in the clients careplan, Any changes in medication or behaviour towards taking medication must always be documented
