Introduction including overview of area inspected and activities taking place This is the report of health, safety and environment inspection carried out at Abudhabi Financial Center project on 31 Jan 2012. During my inspection I have covered the construction site, site office, yard, work shop, welfare facilities, and store. Overall the construction site found good and attitude towards the health, safety and environment is satisfactory up to some extent. A variety of works like scaffolding erection, working at height, confined space works, concreting, hot works, lifting operations, concrete breaking works was going on there in the site. This report addresses the work related health, safety and environmental issues found during my inspection. Some of them need extreme consideration. All observations included in the appendix. Executive Summary The issues need top priority and considerations are unsafe working platform, unsafe scaffolds, confined space entry, unsafe lifting gears, absence of competent scaffolder and rigor, unsafe and damaged electrical distribution boxes, poor cable management, machine without guard, lack of emergency switch, lack of sufficient personnel protective equipments like goggles, dust mask, earplug and ear muffs etc. The issues need urgent consideration and rectification given below. This includes third party training for scaffolders and rigors. Approximate cost for the training and rectifications are noted below. Main findings of the inspection 1) Observation no.1 and 13 Unsafe scaffolds and incompetent scaffolders Unsafe scaffolds and incompetent scaffolders are one of the major issues. The unsafe working at height may result fall from height of workers leads to serious injury or even fatality. Failure to providing safe work places, and allowing incompetent worker for high risk work is the breach of Municipality health and safety codes of practice ministerial order (32) article 13, and article18. All scaffolds should be safe and erected by competent scaffolders. For this we need to give scaffolding training to some workers by third party. 2) Observation no. 7 unsafe lifting operation and incompetent rigors. Unsafe lifting operation going on with damaged web slings and incompetent rigor found in the site. This is because of the lack of the competent rigor. This unsafe lifting practice breached the Municipality health and safety codes of practice, ministerial order (28). In compliance with law and codes of practice we need to arrange competent rigor and good lifting tackles. For the third party training for rigor only cost 100 Dhr/ person. 3) Observation no.21 and 23 damaged electrical distribution box and poor cable management. There was damaged electrical DB and poor cable management found in the site. Electrical connections and equipments should be safe. Failure to provide safe electrical connections and equipments are the breach of Municipality Health and safety codes of practice, ministerial order (32) article 28. In compliance of the law and codes of practice we need to provide new and safe electrical DB. The cost is 300 Dhr/ one piece. For avoid the tripping hazard by electrical cables we need to provide proper lead wire stands. The cost is 10 dhr/ one piece. 4) Observation no. 25 bench saw without guard and emergency switch There is no guard and emergency switch found for bench saw. Working on unguarded bench saw may cause in accident lead to amputation or even fatality. Using of such unsafe power tools is the breach of Municipality health and safety codes of practice, ministerial order (32) article 40. In compliance with the law and codes of practice we have to provide safe power tools and equipments. To rectify this unsafe condition cost 50 dhr only. 5) Observation no.14 and 15 lack of proper personnel protective equipments. It was found that the operative not using personnel protective equipments like dust mask, ear plug, ear muffs, and goggles. This may lead to injury to eye, inhalation of dust will cause respiratory problems and hearing loss. Failure to provide proper personnel protective equipments is the breach of Municipality health and safety codes of practice, ministerial order (32) article 45. In compliance with the law and code of practice we have to provide proper personnel protective equipments for workers. The cost is ear plug 10 Dhr/100 piece, dust mask 15 Dhr/100 piece, ear muffs 15Dhr/100 piece. Conclusions The issues requiring urgent priority attached with this report. The workers are the main asset of an organisation. Health and safety of employees and the properties need to secure properly. Unsafe condition and unsafe act may lead to an accident. Accidents may cause injury of personnel or property damage. A serious accident leads to legal action, fine, civil claim and loss of reputation of the organisation. This may result in the business loss to the organisation. By maintaining a good health and safety system within the organisation we can preserve our good reputation and business. The Employer has the responsibility of duty of care towards the employee and also need to provide the safe work place and the safe plant and equipment and safe welfare facilities.
