Why do you measure individual body waste' - becouse of medical conditions of the individuals - for a good monitoring of they dietary requiremens - measure and record output and/or bodily waste where this is required by the individuals’ care plans Q. What action do you take in terms of recording and reporting when you discover changes in a client' - access and complete the correct records according a. confidentiality agreement b. legal requirements c. organisational policies, procedures and practices -report accurate and sufficient information to the appropriate people like : senior, nurse in charge , deputy manger Q. How do you deal with clients who don’t want to use sensory equipment/ prostheses' - first explaine the individuals and ensure that they understand why they have to use the prostheses and sensory equipment in particular ways and how they can usefully co-operate -move and change individuals positions of sensory equipment/prostheses in ways which minimise pain, discomfort and friction and maximise the individual’s independence, self , respect and dignity - prior useing sensory equipment/prosthese to individuals you check the care plan and the sensory equipment risk assessments - support individuals to understand and overcome their concerns about the use of sensory equipment and/or prostheses, orthoses and creams, seeking additional support, where necessary Q. What actions will you take when you notice changes in the client condition' - press the emergency button - where necessary, you seek additional support to resolve problems - report health and safety issues to the appropriate people and complete health, safety and security records, according to legal and organisational requirements. Q. What will you do where individuals preference conflict with safe practice' - access support from the appropriate people Q. What are the factors that can cause accident in the work place' - falls; hazards in the environment; illness; disability; weaknesses; sensory and cognitive impairment; frailty. Q How do you understand individuals communication methods and specific needs' -support individuals to use their preferred means of communication and language -communicate with individuals, in a manner, and at a level appropriate to the individuals’ understanding, needs and preferences - use appropriate body language, eye contact and methods of listening that actively encourage individuals to communicate - at the appropriate times within the communication: a. clarify points and check that you understand what is being communicated b. confirm/check that individuals and key people understand what you are sayin Q. what are the factors that can affect individuals to the toilet' - work with the individual to identify the degree of support they need with their personal hygiene, encouraging and enabling them to carry out activities for themselves, wherever possible -support individuals to: a. understand which toilet facilities are available to them b. select the toilet facilities they prefer c. go to, and return from the toilet safely - encourage individuals to wash their hands after using the toilet - move and dispose of body waste discreetly, immediately and in ways that: a. minimise the risk of cross infection b. are respectful of the individual’s personal beliefs and preference Q. What do you do when you have difficulties in supporting individuals with their communication needs, equality and diversity' -seek information and advice about the individual’s specific communication and language needs and preferences -legal and organisational requirements on equality, diversity, discrimination and rights when communicating with and completing records and reports about individuals -support equality and diversity -support the rights of people to communicate in their preferred method, media and language Q What action do you take when individuals have difficulties in understanding' -position yourself so that individuals can understand what you have communicated -use appropriate body language, eye contact and methods of listening that actively encourage individuals to communicate -concentrate, listen and respond appropriately when you are communicating with the individuals Q. How will you as a support worker promote / avoid discrimination' -identify and take appropriate action when behaviours and practice discriminate against individuals Q. What actions will you take when you notice changes in the client condition' -recording, reporting, confidentiality and sharing information, including data protection - report to the appropriate people. Q. What will you do when there is risk you cannot deal with' - press the emergency bell - call for help seek extra support -record and report on incidents and emergencies accurately, completely, within confidentiality agreements, and according to organisational and legal requirements. Q. What will you do where individuals preference conflict with safe practice' -find out about the individual’s needs, wishes and preferences -work to resolve conflicts and if you cannot, you seek extra support and advice to help you meet the individual’s needs, wishes and preferences -cared for in a way that meets their needs, takes account of their choices and also protects them Q. When I observe key changes in the individual’s condition and circumstance -where individuals are not eating and drinking or if they are having unexpected difficulties in doing so -record and report the intake of food and drink according to the individual’s plan of care and organisational procedures and practices. Q. Why do encourage resident to participate in their own personal dressing and grooming - maximise their independence - maintain their privacy - promote the individual’s rights, choices and well-being when supporting them with their personal care.
