Hypothetical Working Agreement BSHS 322 Hypothetical Working Agreement The purpose of my paper is to assess and evaluate a client who as been addicted to drugs for four years. She would like to regain her custodial right to her two children, who are currently being taken care of by her mother. I will give a through assessment of Freydia’s situation. Freydia and I will set goal throughout her sessions, we will also set time lines and evaluation methods. Purpose of Assessment The purpose of assessments is for judgment, analysis, and treatment planning or research to conclude eligibility for a particular service. These evaluations are often done to measure the physical health and well-being, strengths of the clients. Clinicians perform on going assessments to evaluate the progression of his or her clients (Murphy&Dillion2003). Assessment. Client name: Freydia Smith Address: 7890 Maple Street, Greenville, NY 1234 Telephone number: (134) 555-6598 Gender: Female Age: 27 Religion: Baptist Cultural Identity: African-American Sexual Orientation: Heterosexual Relationship Status: Single Employment: Unemployed Education: Associate’s Degree in Early Childhood Education Significant Others: Her mother Mary Smith who is 47 years old, 7 years old Daughter named Hayden, and 9 year old son named Jaden. Presenting Problems: Four year drug addiction to crack cocaine, children removed by CPS, and unemployed Biopsychosocial stressors: Single and trying to take care of two children by herself, and could not keep a job due to inadequate child care for her two children, peer pressure, drug infested neighborhood. Client’s strengths: Freydia is very intelligent and great with children. Family Background: Freydia Smith is the only child. Her mom is an elementary school, and her dad is professor at a local college. Significant Development History: normal childhood came from middle class family with both parents, from high school at 17 Personal Situations: Single mother with two kids, evicted from her apartment from for lack of payments. Health and Physical Status: Underweight, hallucinations, paranoia, heart problems, and respiratory problems. Cognitive/intellectual capacities and skills: Paranoid, mood swings, anxiety, confusion, and restlessness. Behavioral assessment: Depress ional, schizophrenia, anxiety, irritability, restlessness, confrontational attitude, erratic behavior, insomnia, decrease in appetite and weight. Emotional/affective state: Develops a tolerance for the drug, larger doses are needed to feel “the high”, lose interest in friends and family. Purpose of setting goals The purpose of setting goals is for the clinician to observe what his or her clients wants to sustain or improve. Goals are learned by potential images of how clients and clinicians would like things to be. These goals are broken down into objectives. Goals are then ranked in order of their urgency or importance (Murphy&Dillion). Goal Setting: • Freydia first goal is to admit she has a problem, comes to terms with her addiction. • Enter a drug rehabilitation program. • Successfully finish the rehabilitation program. • Remain drug free for a long period of time. • Attend job counseling. • Find a job • Keep the job for a long period of time. • Attend family counseling with her mother and two children • Attend individual counseling Services or Activities. • Individualized addiction treatment plans • On to one addiction counseling and group therapy. • Lectures and other educational activities • Special groups, such as relapse prevention, anger, eating disorder, and family issues. • Relaxation exercises and recreational activities. • Time for fellowship, allowing time for personal sharing and understanding. • Comprehensive continuing care plans to support recovery after primary addiction. Measures with timelines. Freydia will attend her drug treatment program for a year. She will continue her ongoing assessments after she finished her detoxification program. Freydia will attend her job counseling for six months, then find and maintain a job for, then attend family and individual counseling indefinitely. Contracting and Evaluating Contracts are used to develop working agreements connecting clinician and clients and any important individuals who are vital to these goals and objectives. Contracts should be specific, but flexible at the same time. These contracts should be mutually constructed. The purpose of contracting and evaluating Contracting. I Freydia Smith agree to focus on and met all of the goals that I have set for myself. I will enter a rehabilitation and detoxification center, attend job, individual and group counseling. I will become financially stable so I can be approved for suitable living arrangements for my children and me. I will use the emotional support from my family and counselor to meet all of my goals. I will give consent to a social worker to perform random house. I understand that I must attend our counseling session three days a week, for fifty minutes per session. I will adhere to our payment arrangements until our sessions are completely paid in full. If I am not able to attend our session or make the payment arrangements, I will provide a 24 hour notice for my inconvenience. I am aware that our sessions are to make a better person so I can survive in society. These sessions will assist with my decision making as a mother and member of society. Evaluation method. The best way to evaluate Freydia’s progress is to perform assessments during each of Freydia’s sessions. This way I will be able to observe Freydia’s progression so she will be able to attain her next goal. Next I will see if Freydia will be capable of maintaining her job so she save money to get a suitable place for her and her two children. If she is successful with this, then I perform the family counseling, and observe how she interacts with her children. I will ensure that Freydia is emotional and financially stable to meet her children’s basic needs. If all of Freydia’s goals are met, then I will place her on a trial period with her children, and perform random home visits to ensure her stability. I have stated the purpose of my paper. What goals that I have set for Freydia to meet, and the duration to which these goals are to be meet. I have drawn up an informal contract that Freydia and I must uphold throughout our session. I have stated what goals Fredydia must accomplish to get and keep her two children. I do believe that our contract is clear and comprehensible. Refernces Murphy, B. C., & Dillon, C. (2003).
