ALLEGED FOOD POISONING REPORT
In the event of a complaint of food related illness the Food & Beverage Manager, or Senior Manager on duty, should deal with the matter and take action as follows:
1. Complete the form below – one for each person reporting illness
2. Inform Regional Environmental Health and Safety Manager of the incident
3. OBTAIN SAMPLES TO SEND TO NOMINATED LABORATORY, IF INSTRUCTED. If samples are needed, they must be frozen and placed in a leak proof, sterile container and then packed in ice in another leak proof container to await collection by the laboratory. A copy of this form should be enclosed with the sample.
4. Refer to procedures in “Dealing with Food Complaints”
