A nursing diagnosis can be defined as clinical reference point used in coming up with a solution to a person’s issues ,which is in this case a health perspective . As a reference point it provides for accountability on the part of the nurse in selection of the best options to achieve a desired outcome (NANDA –I 2009).
Chronic Self esteem
This is a psychological disorder where a person has developed a continuous habit of having negative evaluation about themselves.
Typically a person with chronic self-esteem never focuses on their positive qualities at any one time but always downplaying them. It is also worth noting that the person will tend to judge themselves inferior to their peers . Often they tend to use no positive words to describe themselves.
When given an undertaking they will blame themselves when things go wrong instead of taking into account other things over which they have no control such as the actions of other people or economic forces.
They will always think they are being flattered when a person compliments them . Their minds have been set to always receive negative responses.
In dealing with self-esteem the first and most important thing to do is to change the environment of the client. This would entail having the client to reside at my institution. This is important because the environment according to the proven research plays a big role in ones self-esteem .
The next step would be to talk to the client on his or her positive attributes .This can be enhanced by talking about ones attributes and being enthusiastic about them . This makes a client to also have a high sense of their attributes. Telling the client some of testimonies of people that believed in themselves even when their missions seemed impossible would also help develop a spirit of self believe as they will have a character to benchmark on.
Using science as a weapon, exercising has been known to be a good boost to the brain for all kinds of things but especially in combatting depression and helping you to feel good. Targets need to be step by step, such as walking around the institution. If the client comes from a place with endowed resources such as a gym, I would recommend for enrollment. If the client is fond of water swimming would be ideal as it also achieves the same result as going to the gym.
Anxiety
Anxiety disorders are of many types . The main focus although will be put on panic disorder in this writing.
Anxiety is said to be normal to humans . This is because humans have emotions . Many people feel anxious especially in certain circumstances such as when doing an exam ,when making a decision that has a far reaching consequence .Anxiety disorders, however are not normal as they affect one’s ability to lead a normal life.
Panic disorder
Panic disorder is a condition where by one has had a series of repeated panic attacks.
A panic attack can be defined as a sudden episode characterized by fear and apprehension . It is usually for a short period of time.
Some the signs exhibited by my patient to indicate the disorder include increased heart rate suddenly without having any frightening thing in sight. The patient is sweating even with cool temperatures and has no any other ailment . It is often to see the patient shaking especially the hands .
Panic disorder is normally caused by environmental factors. For example strict parents who are overly cautious about their children’s performance with a decline in performance associated with some form of punishment.
Some dynamics have also shown that in one way or the other panic can be hereditary.
In order to address panic I would use Cognitive behavioral therapy: This helps in changing the way of thinking that has been accustomed to panic trauma.
Encouraging the client to avoid some things such as watching horror movies that hence lead them to develop panic over a certain thing watched
Fear
This is a disorder that can be described as
Some of the signs shown by my client as a prove to the disorder include blushing,
Having sweaty palms and difficulty in concentration especially when holding a conversation.
The first step in containing fear would be avoiding the situations in which fear occurs .This is a short term measure that may help .
The second step has to deal with what actually fear is . Once the client has full knowledge ,they would start to act as if it does exist. Once people understand what fear really is, they can begin to act as if it doesn’t exist. I would urge the client to examine his fear rather than merely reacting to it.
The third step towards finding a solution has to do with showing my client how to exert some control over their fears through their action.
.Once people understand what fear is, the next step is showing them that, through their actions, they can exert some A over it
I would also encourage my client to always speak about his fear as a step towards eliminating it
