Interview Informed Consent
 

Description and Purpose of the Research:

You are being asked to participate in an interview on the research topic described below.

To fulfill the requirements of a college course, ____________________,  ________________

is collecting information on an issue [about the countryside education]
         Description of Issue
and is interested in the factors contributing to the low educational level in the countryside.

The purpose of ethnographic research is to discover how people in a particular community understand their own lives, their behaviors and their cultures in their own terms. For example, an ethnographic researcher may attempt to discover how the members of a sorority make sense of their group and define the rules they live by. 

Your information will become part of a general pool of data on this issue. A pseudonym will be used where necessary when referencing your individual material, otherwise your information will be included in a general, anonymous pool of informants.

Voluntary Participation:

You may choose to refuse to continue this interview at any time, may choose to have your responses destroyed and be unavailable for the research. You will experience no penalty from your refusal.

Confidentiality:

In this study, every effort will be made not to reveal personally identifiable information
in publications based on this research. To accomplish this, no records will be created or
retained that could link you to personally identifiable descriptions, paraphrases, or
quotations. Your actions or things you say may be presented without specific reference
to you, referenced only by pseudonym, or combined anonymously with the actions and
words of other participants. The interview will be audio recorded with your permission and the audio files are transcribed will be erased after transcription. 

Risks and Benefits:
There is no more than the minimal risk from everyday life. Potential benefits outweigh the risks. Benefits include, primarily, making available to the community greater knowledge on a particular research issue.

Consent Statement:

I, having read and understood this consent statement, have been fully informed by the researcher of the purpose of this research, and I consent to being interviewed and allowing my responses to be used in this research. I am 18 years of age or older.
